The below is a special request by the customer to either move an item that has damage or to perform an action by the customer’s request that can lead to damages.
Consent to Perform
By signing below, you authorize and give consent to move the item photos and or Request Notes above, and that Local Motion, or any member of the Local Motion team, will not be held liable for any damages, delays, or added cost that could potentially inquired by following the request.
